Cidade Saudável ACS: A Health and Fitness App for Community Health Agents

Cidade Saudável ACS is a system that can be acquired by municipalities and installed for monitoring by the Health Department, and used by Community Health Agents (ACS). This Android application allows the replacement of paper forms, used by Community Health Agents, with electronic forms filled on tablets. The Cidade Saudável ACS works both online and offline. Some of its key features include individual registration, household registration, home visits, among other frequently used forms. The app also obtains coordinates for patient geolocation and provides statistical data analysis. It is developed by Sysvale Softgroup Ltda and is licensed for free.

It is important to note that when using the test mode by clicking on the "Take a Test" button on the login screen, the registered forms will not be saved on the server and will not be submitted to the PEC. To use the application, you must be linked to a health department that uses Cidade Saudável - ACS module.
